Doc 4, the fiction with Luca Argentero departed from 1 October on Rai 1

Andrea Fanti’s return to the Ambrosian Polyclinic opens a new phase for Doc – In your hands. After moving away from the hospital to stay next to Agnese, Andrea returns to the department and finds a very different situation than he had left: Giulia Giordano became primary, the budget of the structure is in red and the new health director Stefano Campigli has imposed a series of cuts to avoid the failure of the hospital. The fourth season of Doc – In your hands debuted on Rai 1 and RaiPlay from Thursday 1 October 2026 and consists of 16 bets of 50 minutes divided for 8 weeks (press two available already on RaiPlay). The TV series has had a great international feedback, in the United States the adaptation is already to the third season.

La trama della stagione

Andrea Fanti had left the Ambrosian Polyclinic at the end of the third season to devote himself to Agnese, his former wife, who had been diagnosed with a rare disease. It has been a little more than a year and Andrea returns to find the department in the hands of Giulia Giordano, in the meantime become chief of Internal Medicine.

The new chief had to face a difficult financial situation and the consequences of the decisions of the new health director Stefano Campigli, called to balance the Ambrosian accounts and to prevent the purchase of the structure by a private holding company. The cuts led to the reduction of staff and decency beds, partially replaced by an outpatient intended to allow the department to follow a greater number of patients.

The new organization does not convince Andrea, who returns to fight for his idea of medicine: more doctors, more nurses and more time to devote to patients. His position inevitably contrasts him with Giulia and Campigli, but behind this battle there is also something personal. Andrea struggles to accept the changes that occurred during his absence and tries, at least in part, to rebuild the past that he lost.

This story adds the arrival of Matteo Pellegrini, a new specialization that shares with Doc the passion for medicine and in which Andrea recognizes something of himself. The two also find themselves investigating some suspicious deaths in the Ambrosian: behind those deaths there could be a precise design and one of the main questions of the season concerns the possible involvement of Campigli.

La trama delle puntate

1° ottobre

4×01 everything flows: After the waiting period, Andrea returns to a deeply changed department. The cuts imposed by the new health director and Giulia, who became primary, reduced the number of doctors and nurses. Doc tries to cope with Richard, but the pressure on staff is increasing and a mistake risks having very serious consequences for one of the patients.

4×02 here and now: The climate in the ward remains tense. Martina must confront Richard’s hostility, who did not accept his decision to leave for London, while Matteo struggles to join the team after being accustomed to working alone. Everyone also awaits the outcome of the internal investigation into the error committed in the department. Meanwhile, Doc receives an anonymous letter that seems to open an even more disturbing scenario.

The fourth season sees Luca Argentero as Andrea Fanti and Matilde Gioli as Giulia Giordano. Pierpaolo Spollon, Marco Rossetti and Elisa Di Eusanio, together with Laura Cravedi, Elisa Wong, Alessandro Piavani and Gabriele Falsetta, also returned.
